We are currently seeking experienced Part Time and Per Diem Registered Nurse to manage a caseload of patients in New Haven county and nearby area.
This position would be with our Visiting Nurses Agency and provide care to our patients at home.
Working with a team of dedicated provider, the RN Homecare Nurse will manage patient care in partnership with Primary Care Physicians and other community providers to bring comprehensive healthcare directly into the home.
The goal of assisting patients and their families to better manage care as well as complex and chronic illnesses, and hereby decreasing unnecessary hospitalizations and ER visits.

Qualifications:
· Active professional Connecticut Registered Nurse license
· Ability to work independently and as a part of team
· Excellent communication and patient care skills
· Manage caseload of patient from Admission to Discharge
· Experience in Home Care preferred
· Solid patient assessment skills
· Comfortable with using EMR
· Great critical thinking skills and ability to triage patients and problems
· LPN supervision
· HHA supervision
